JourneyWell Use & Participation Acknowledgement
JourneyWell is a recovery support app designed to help patients and care partners capture what matters, replay clinician guidance, stay engaged between visits, and see progress over time.
What you are acknowledging
By choosing to use JourneyWell, you understand and agree that:
Support tool, not medical care: JourneyWell is a care support tool and does not replace medical care, rehabilitation, mental health care, or the professional judgment of your care team.
Not for emergencies: JourneyWell does not provide emergency services. If you believe you may be experiencing an emergency, call local emergency services or contact an appropriate healthcare professional directly.
AI-supported content: JourneyWell may use artificial intelligence to help structure clinician guidance and other information you provide into summaries, plans, reminders, or progress updates. AI-generated content may be incomplete or inaccurate and should not be considered medical advice or a substitute for guidance from your care team.
Patient-led sharing: Patients control who they invite to their Care Team and can change or remove access at any time.
Care Team collaboration: If you invite a care partner or clinician to your Care Team, they may view or contribute information based on the access available to their role, including session recaps, plans, reminders, observations, or resources.
Recording and constent: If you record another person using JourneyWell, you are responsible for obtaining any permission or consent required by applicable law. Your clinician or care organization may also require its own release, recording consent, or other authorization.
Use your judgment: You are responsible for how you use information available through JourneyWell and for contacting your care team when you need medical or clinical guidance.
Consent
By agreeing, you confirm that you understand JourneyWell's role as a care support tool and choose to use it to help carry clinician guidance forward between visits, including collaboration with your Care Team when applicable. You also understand that information you provide may be processed using AI-supported services to provide JourneyWell features.
